Brochure Ordering
The Lyme Disease Association provides the following brochures:
Effective: November 1, 2008
In order to defray shipping and handling costs associated with mailing LDA
brochures, we need to charge a nominal fee on orders of more than 200. Please refer to the chart below for fee schedule.
FEE SCHEDULE
1—200 brochures FREE
201—400 brochures $11.00 s&h
401—800 brochures $15.00 s&h
801 + brochures $27.00 s&h
* Brochures can be sent in any combination (e.g., 100 Lymer Primer,
100 ABC’s, & 50 tickmarks = 250 brochures; total cost = $11.00).

Please email macavitypl@hotmail.com, with a subject line of "LDA - Order Brochure". Please provide your name, address, telephone, quantities of which brochures and date needed by (if for an upcoming event).
Online: ordering and payment of brochures over 200 (shipping & handling)
Please pay shipping & handling below with your order. Write in date needed by and combination of brochures needed in Special Instructions to Seller (e.g. 100 LPs, 200 ABCs, 100 tickmarks = 400). That order is $15 shipping.

Make payable to Lyme Disease Association, PO Box 1438 Jackson, NJ 08527. Use shipping & handling payment schedule above and list combination wanted (e.g. 100 LPs, 200 ABCs, 100 tickmarks = 400, which is $15).
All shipping & handling must be prepaid. Allow 4 weeks for delivery.
